Obtaining a Certificate of Registration

For a veterinary certificate of registration, submit the following forms with the appropriate fee:

Fee Schedule for New Certificates of Registration

The table below outlines fees pertaining to the issuance of a single certificate of registration for veterinary use.

For a complete listing of all types of radiation machines and their associated fees, please see 25 TAC, §289.204.

An updated certificate of registration will not be issued based on payment of biennial fees.

Veterinary Radiographic, Computerized Tomography (CT), Fluoroscopy & Accelerators $290
Reciprocity Out of State Reciprocal Recognition $290

Each initial application for a certificate of registration shall be accompanied by a non-refundable fee.

Fee payments by check or money order shall be made payable to Texas Department of State Health Services.

Additional Use Locations

If the certificate of registration has additional authorized use sites, the registrant shall pay an additional 30% for each additional use location.

 

Biennial Fees for X-Ray Registration

Biennial fees must be paid every two years in order to continue operating X-ray and laser devices. The fee covers all direct and indirect costs associated with licensing a company or individual.

Amending a Certificate of Registration

*Equipment, name, address, and RSO changes* can be completed online using the Regulatory Services Online Licensing System. For additional information visit the Online Application Instructions page.

To amend a veterinary registration, submit RC 226-2. Describe the change, reference the registration number, and include any additional information needed to process the request.

Name and/or Ownership Change - See Regulatory Guide 8.2: Guidelines for Issuing Certificates of Registration Due to a Change in Name and/or Ownership.

Radiation Safety Officer Change - Complete form RC 42-R.


Terminating a Certificate of Registration

To terminate a registration, complete and submit RC 17-R Termination Form.


Disposing of an X-Ray Unit

Title 25 of the Texas Administrative Code (TAC) §289.233 and §289.229 requires each registrant to have and implement written operating and safety procedures. Veterinary Accelerator

For registrants with an accelerator, Form RC 229-01 Additional Information for Accelerator Registration will help you in providing the necessary shielding and room design information.


Mobile Veterinary Services

Authorization must be obtained prior to providing mobile veterinary services.
