Obtaining a Certificate of Registration
For a certificate of registration for dental radiation machines, submit the following forms with the appropriate fee:
Fee Schedule for New Certificates of Registration
The table below outlines fees pertaining to the issuance of a single certificate of registration for use in dental radiography.
Fees are established in 25 Texas Administrative Code §289.232, and in 25 TAC, §289.204.
An updated certificate of registration will not be issued based on payment of biennial fees.

Each initial application for a certificate of registration shall be accompanied by a non-refundable fee.
Fee payments by check or money order shall be made payable to Texas Department of State Health Services.
Additional Use Location
If the certificate of registration has additional authorized use sites, the registrant shall pay an additional 30% for each additional use location.
Biennial Fees for X-Ray Registration
Biennial fees must be paid every two years in order to continue operating X-ray devices. The fee covers all direct and indirect costs associated with licensing a company or individual.

Authorization must be obtained prior to providing mobile dental services.

Operating and Safety Procedures
Title 25 of the Texas Administrative Code (TAC) §289.232 requires each registrant to have and implement written operating and safety procedures. The following Regulatory Guide can be used to help assist registrants in developing procedures unique to individual dental facilities. For mobile services, include specific procedures unique to your mobile operation.
