It is easy to request health data from Texas DSHS. Requesters needing data for research must complete a data-sharing agreement (DSA) for each data type requested. A DSA outlines the conditions in which DSHS can share health data with outside organizations. Recipient(s) agree to the terms and conditions, which ensure confidentiality and prevent identification of individuals. Data may not be disclosed, linked, or used to attempt to identify individuals in the file. Members of the general public requesting data for their own use do not need to sign a DSA. Members of the media must contact the DSHS Press Office to request data.

Texas has an opt-in Immunization Registry. If you or your parent opted into the Texas Immunization Registry, you can contact us to get a copy of your immunization records. Adults must opt back into the registry before they turn 26 years old to ensure that their records from childhood not archived.
