Hospital Discharge Data Public Use Data File

Sections 108.013(c)(1) and (2) and 108.013 (g) of the Texas Health and Safety Code (THSC) prohibit the Texas Department of State Health Services (DSHS) from releasing, and a person or entity from gaining access to, any data that could reveal the identity of a patient or the identity of a physician unless specially authorized by the Act. Any effort to determine the identity of any person or to use the information for any purpose other than for analysis and aggregate statistical reporting violates the THSC and this data use agreement. By virtue of this agreement, the undersigned agrees that the data will not be used to identify an individual patient or physician. Any questions about the data must be referred to DSHS only. Product support is not provided by DSHS. The data are protected by United States copyright laws and international treaty provisions.

In this data use agreement, the person who downloads the data is referred to as the "licensee". The licensee gives the following assurances with respect to the use of the DSHS Hospital Discharge Data sets:

  • The licensee will not release nor permit others to release the individual patient records or any part of them to any person who is not a staff member of the organization agreeing to these requirements, except with the written approval of DSHS;

  • The licensee will not attempt to link nor permit others to attempt to link the hospital stay records of patients in this data set with personally identifiable records from any other source

  • The licensee will not release nor permit others to release any information that identifies persons, directly or indirectly;

  • The licensee will not attempt to use nor permit others to use the data to learn the identity of any physician;

  • The licensee will not nor permit others to copy, sell, rent, license, lease, loan, or otherwise grant access to the data covered by this Agreement to any other person or entity, unless approved in writing by DSHS;

  • The licensee agrees to read the User Manual and to be cognizant of the limitations of the data;

  • The licensee will use the following citation in any publication of information from this file:

  • Texas Hospital Inpatient Discharge Public Use Data File, [quarter and year of data]. Texas Department of State Health Services, Austin, Texas. [date of publication];

  • The licensee will indemnify, defend, and hold the DSHS, its employees, and contract vendors harmless from any and all claims and losses accruing to any person as a result of violation of this agreement; and

  • The licensee will make no statement nor permit others to make statements indicating or suggesting that interpretations drawn from these data are those of DSHS.

The licensee understands that these assurances are collected by DSHS for each set of data acquired by the licensee to assure compliance with its statutory confidentiality requirement. Recipient agrees to the foregoing restrictions and acknowledges that the knowing or negligent release of data in violation of Chapter 108, Health and Safety Code, is punishable by a civil penalty of up to $10,000 under section 108.014 and is a state jail felony under section 108.0141.

The inpatient Public Use Data File (PUDF) for 2006, 2007, 2008, 2009, 2010, 2011, 2012, 2013, 2014, 2015, 2016, 2017, and 2018, in fixed-length format, is available for free download.  By clicking the agree button, you agree to the above conditions, and you will be able to download this data.  To request 1999-2005 data, you must submit a free inpatient order form.

Technical Updates

An update has been released for the third quarter 2016 Inpatient Status Reports. The facilities listed in the update were assigned an erroneous status. The update contains the correct reporting status for each facility.

  • Third quarter, 2016 - Update in Excel form 

An update has been released for the third quarter 2012 file. The records identified in the update are duplications of Outpatient claims and should be deleted from your copy of the Quarter 3 2012 Inpatient Public Use Data File (PUDF).

Fourth quarter 2011 Inpatient computer disks (4Q2011 Inpatient CD) did not include the Facility Type files. December 17, 2012.

Records for additional discharges in Quarter 2, 2010 have been found to be duplicated in the data for Quarter 3, 2010. The following update includes the Record ID for those duplicate records that should be deleted from your copy of the Quarter 3 Public Use Data File (PUDF). September 7, 2011.

Charges records for some discharges were inadvertently omitted from the Second quarter 2010 PUDF. These records should be added to your copy of the Quarter 2 2010 Public Use Data File (PUDF).

Records for additional discharges in Quarter 4, 2009 have been found to be duplicated in the data for Quarter 1, 2010. The following update includes the Record ID for those duplicate records that should be deleted from your copy of the Quarter 1 Public Use Data File (PUDF). August 19, 2011.

Records for some discharges in Quarter 4, 2009 were inadvertently duplicated in the data for Quarter 1, 2010. The following update includes the Record ID for those duplicate records that should be deleted from your copy of the Quarter 1 Public Use Data File (PUDF). May 18, 2011.

Records for some discharges in Quarter 1, 2009 were inadvertently duplicated in the data for Quarter 2, 2009. The following update includes the Record ID for those duplicate records that should be deleted from your copy of the Quarter 2 Public Use Data File (PUDF). September 9, 2010.

Updates to correct missing uniform physician identifiers have been released for the second, third and fourth quarter 2007 Public Use Data Files (PUDF) and the first, second and third quarter 2008 PUDF. The updates include uniform identifiers for the Attending Physician and Operating Physician fields, ATTENDING_PHYSICIAN_UNIF_ID and OPERATING_PHYSICIAN_UNIF_ID, that were set to missing in the original files. An update has been released for the second quarter 2006 Public Use Data File (PUDF). It updates county codes, ZIP codes and Public Health Regions that were set to missing in the original file. Use the update to correct your copy of the PUDF. For more information please contact THCIC Staff.

An update has been released for the third quarter 2004 Public Use Data File (PUDF). The update includes those records for which the 3M APR-DRG grouping was corrected. This correction was necessitated because some records were not grouped with Version 20 of the APR-DRG Grouper. The files include the Record ID and four additional data elements: APR-DRG code, APR-MDC, Severity of Illness, and Risk of Mortality. Use the update to correct your copy of the PUDF.

Updates have been released for the 2003 PUDF. The updates include Patient Status codes for those records where the patient status was incorrectly indicated as invalid. The files contain the 12-digit Encounter ID and the 2-digit Patient Status Code. Use the files to update your copy of the PUDF. September 23, 2005

The User Manuals for 1999, 2000 and 2001 have been updated to correctly document the data field ADMIT WEEKDAY. The values for this data field should be interpreted as:

  • Monday
  • Tuesday
  • Wednesday
  • Thursday
  • Friday
  • Saturday
  • Sunday
