Survey Deadline Extended for Chambers County Community Health Assessment

People wanting to participate in the Chambers County Community Health Assessment now have until Feb. 13 to complete the survey. People who live or work in Chambers County also can attend two additional community forums on Feb. 3 and Feb. 10.

The Texas Department of State Health Services (DSHS) and several local health departments are assessing community health in Chambers County, focusing on education, prevention, care and treatment services for physical health, mental health and substance abuse.

Participants can complete the anonymous survey in English or Spanish online at www.dshs.state.tx.us/chambers. They also can complete a paper survey available at the community forums or from Tiffany Carrethers at (713) 767-3005 or by e-mail to tiffany.carrethers@dshs.state.tx.us. As of Jan. 29, 335 people have completed a survey.

The additional community forums will be held from 6 p.m. to 7:30 p.m. Feb. 3 and Feb. 10 at the American Legion Hall, 1704 S. Main St. in Anahuac. Forums already have been held in Baytown, Winnie, Mt. Belvieu and Anahuac.

Interpretation services and special accommodations may be arranged by calling Tiffany Carrethers at (713) 767-3005 or sending an e-mail to tiffany.carrethers@dshs.state.tx.us at least 72 hours before the forum.

An assessment report based on the survey and forum results should be available by April 10.
